A paneled-over emergency exit door plug on an Alaska Airlines Boeing 737 MAX 9 blew out shortly after takeoff from Portland, Oregon. 171 passengers and 6 crew aboard. No serious injuries. NTSB investigation found lapses in Boeing manufacturing and FAA oversight led to the incident.
